In the Purpose of supporting clients to live safely, comfortably with dignity while promoting independence progressively (Giraneza Home care Ltd) is looking for a qualified individual to fill full the responsibilities in the Home Care Giver support and coordination both at hospital and community where necessary. With the following responsibilities:
Coordinates resources and personnel to facilitate successful completion of Giraneza Home care ltd with combination of providing the following patient care support bundles:
1. Activities of daily living
2.Dressing and undressing
3.Toileting and continence care
4.Feeding and nutrition support
5.Mobility and transfer assistance
6.Positioning and pressure injury prevention
7.Sleep and rest support
8.Safety and environmental support
9.Promoting independence and dignity
10.Documentation and reporting
11.Ensure compliance with standard operating procedures for Giraneza Home care ltd
12.Provides individual education sessions with Giraneza Home care ltd staff to ensure compliance with process as it related to the company.
